STUDENT RECORDS - PERSONAL INFORMATION CHANGES

Name, Address and/or Telephone Number Changes

The Admissions, Records and Recruitment Office should be informed of all changes in the student’s legal name, mailing address and/or telephone number. Southwest is not responsible for a student’s failure to receive official information due to the failure to notify Admissions, Records and Recruitment Office personnel of any changes. Address changes may also be completed on My.Southwest. A copy of court documents must be submitted for a legal name change other than a change. In the event of marriage or divorce, a marriage license or divorce decree must be submitted for a name change.
