Student Records-Personal Information Changes
The Admissions and Records Office should be informed of all changes in the student's legal name, mailing address and/or telephone number. Southwest is not responsible for a student's failure to receive official information due to the failure to notify the Admissions and Records Office personnel of any changes.
To change the name or mailing address, students must complete a "Change of Record" form in the office of Admissions and Records. A copy of legal documentation must be submitted for a name change. In the event of marriage or divorce, a marriage license or divorce decree must be submitted for a name change.
